Our senior nurses at HCC possess a diverse skill set that goes beyond general nursing care. They have developed a deep understanding and knowledge in specialist areas such as wound management, neurological conditions including Parkinson’s disease, Dementia, Autism, Ehlers-Danlos Syndrome, Stroke, ABI (Acquired Brain Injury), Cerebral Palsy, spinal cord injuries, and adult and pediatric continence care. This extensive expertise enables our nurses to provide a more specialized and tailored service to our clients, catering to their unique needs and conditions.

Each referral is traiged to the most appropriate nurse within the team and throughout the clients journey with us they have the benefit of more than one person managing them through collaboration of the dedicated team.  Each service is individualised as the care and management required for each person is unique in that their situation and needs.

In-Home Clinical Nursing Care

No-one wants to be in hospital and at HCC our aim is to keep you at home with skilled nurses to support you at home with your clinical care needs.

Our team of Registered Nurses, Clinical Nurse Consultants and Nurse Practitioner leading them aim to provide continuity of care with the same nurse and ongoing support.

Whilst nurses do have super powers, we believe in providing nursing care with guidance from Nursing specialists. Hence within our team we have wound specialists, continence experts, paediatric continence advisors and Nurse Practitioners specialising in neurological conditions. Each referral is triaged to the most appropriate nurse within the team based on their needs and throughout the clients journey with us they will have the benefit of more than one person managing them through collaboration of our dedicated team.

We can undertake general nursing services including assessment of nursing needs, basic and complex wound management, ongoing catheter changes and management, Peg management and ongoing or establishment of bowel management regimes.

Continence Assessment, Management, Reporting and In home Trial of Void

Many adults and children experience incontinence which can have a significant impact on their quality of life.  Our skilled Continence advisors and Nurse Practitioner (specialising in management of the Neurogenic bladder and bowel) have extensive experience in assessing, managing and reporting to optimise the individuals continence management.

Our Continence assessments are conducted by qualified continence advisors who are trained in continence management of adults and/or children.  We come to you to establish your continence goals and current situation and make recommendations on how continence can be optimised.  These reports form the basis of a continence plan which are individualised to each client.  Annual costings of consumables is incorporated within the report for planning of consumables funding requirements within NDIS plans. 

Neurogenic Bladder and Bowel management is also a specialty of ours as it needs to be considered in a different context to general continence with extensive knowledge regarding the anatomy and physiology of the nervous system to ensure individuals are managed in the most effective and safe way.

Our Paediatric Continence Clinical Nurse Consultant is skilled in managing common paediatric needs such as nocturnal enuresis (bed wetting) and implementing toileting plans to optimise this.

Nursing Care Plans

Having individualised nursing care plans for clients working with multiple service providers and support workers is essential to ensure quality care.  HCC have skilled nursing staff who can develop, implement, and maintain these care plans for clients as required.  HCC will work with the client to identify the need for care plans relating to various systems, Common care plans requested but not limited to include

Education and Training

HCC are passionate about education and training of the client, family and their support team in the community to ensure the delivery of quality care and prevention of complications to maximise quality of life.  Informal education is a part of our general nursing service when visiting our clients, however often formal education is required to meet the needs of the care team.  We have skilled nursing staff who will discuss with the client their individualised education and training needs and formulate a plan to ensure these are met both initially and ongoing as required.  Common requests for education include but not limited to
